1. Latch up can occur when the prescribed power-up sequence (VM_DD->VM_SS->GND->VA ->VB) is not followed.
2. A voltage greater than |VM_DD - GND| or |VM_DD - VM_SS| (if dual-supply) applied across terminals A-B, W-B, or W-A can cause failure.
